Work Permits

 

In order to obtain a work permit follow these directions.

 

1)  Please print this form(Click Here). 

 

2)  Get all the necessary signatures from your parents and employer.

 

3)  Bring the completed form to Mr. Stuart at lunch Tuesday-Friday, in Room P-5, to be processed.
