Sequoia Union High School District (SUHSD) has partnered with Frontline for the Daily Health Check, designed to provide

a daily health and exposure check for the school community. More Health & Safety measures are on SUHSD website.

** IF YOU ARE EXPERIENCING SYMPTOMS OF COVID-19, ARE CURRENTLY DIAGNOSED WITH COVID-19, OR ARE IDENTIFIED AS A CLOSE CONTACT TO SOMEONE WITH COVID-19, PLEASE REMAIN OFF-SITE UNTIL CLEARED.

Health Screening Requirement

All SUHSD students on campus must log in to account and complete the daily health check through Frontline BEFORE entering any school/facility each day - this is in the “To Do List”.
Take a screenshot after you complete the questions, or keep your app open so “Health Check Cleared” is visible.
Show “Health Check Cleared” to school staff when student(s) arrives at school.

After extensive work and planning for the last several months, the District and Sequoia District Teachers Association have reached a memorandum of understanding for a return to in-person instruction on April 5, 2021.

Students returning to campus need to:

- - Review the Return to Campus Webinar and Slides
  - Complete this form to confirm understanding
  - Log into your Infinite Campus account, click on schedule, and identify the room number for each of your classes.
    - As a reminder, your login in information for Infinite Campus is as follows:
      - Username = Student ID number. (Example: 768345)
        
        Password = First name initial + Last Name Initial + 8 digit birthdate (Example: jk090302)
      - This log-in is also what you'll use to access the student wifi if needed
    - Check this Map for Entrances and Zones
    - If you have a technological need for a Chromebook, hotspot, or something else, make an appointment with our IT department.
    - Set up your Frontline Account for Daily Health Screenings.
