Athletic Clearance for 2020-2021

 
Friday, May 1st, SportsNet will be open for the 2020-2021 season. All Athletic Clearance forms must be done online. 
 
Athletic Forms are due no later than:
 
Cross Country, Football, Water Polo, Volleyball (Boys & Girls): December 7, 2020
 
Badminton, Soccer, Tennis (Boys & Girls): February 15, 2021
 
Swimming & Diving: March 1, 2021
 
Wrestling, Basketball, Baseball, Softball, Golf, Lacrosse, Track: March 8, 2021

Insurance is not accepted for physicals. Payment is due at time of visit. TB testing may be covered by some insurance plans; please check with your insurance to determine your coverage.

Consult with families about students needing health care
Assist with questions about special education, accommodations in the classroom for students with chronic Illness, home teaching and medical resources
Health counseling
